I will personally meet with you via video or phone call to assess your needs and confirm how I can best support you.

Following our meeting, I will create a custom plan designed to help you achieve your desired outcome. Before work begins, you'll complete required documentation, including HIPPA form, Release of Information, Disclaimer, and any additional forms required by the facility or office.

Once the paperwork is complete, I will begin my review and prepare a detailed summary of my findings. You will have the opportunity to review this summary before we schedule a follow-up appointment to discuss next steps.
